About the Role:
Slough are seeking an experienced Locum Social Worker (Level 7) to support the Proportionate Care Project. This vital role involves reviewing existing care packages, specifically those flagged by Occupational Therapists as part of this innovative project aimed at delivering smarter, more sustainable care.
Key Responsibilities:

• Conduct comprehensive care package review assessments in line with project recommendations
• Identify and implement appropriate care package reductions without compromising service quality
• Work collaboratively with OTs, service users, and their families to ensure proportionate, person-centred care
• Accurately record assessments and decisions within required timelines
• Ensure all work adheres to current legislation and best practice

Requirements:

• Qualified Social Worker with current Social Work England registration
• Proven experience in care package reviews and reductions
• Strong understanding of the Care Act 2014 and strength-based approaches
• Excellent time management and organisational skills
• Ability to work independently and manage a high-volume caseload
