QSW – ADULTS SOCIAL WORKER
Job Description
About the Role:
This is a unique opportunity to join a newly established Mental Health Social Work Discharge Team (MHSWDT)—part of a ground-breaking pilot project in partnership with Sussex Partnership NHS Foundation Trust. You will play a key role in supporting timely and safe hospital discharges for people with severe and enduring mental health conditions, working directly with wards and within a dynamic multi-disciplinary Discharge Hub.
Your responsibilities will include:
- Supporting discharges from mental health hospitals (acute & community-based)
- Conducting Care Act assessments, Section 117 aftercare planning, safeguarding inquiries, and Mental Capacity Act work
- Developing strength-based support plans in partnership with individuals, carers, and partner agencies
- Managing a caseload and prioritising complex needs and risks
- Acting as a professional advocate and upholding Social Work England standards
- Providing informal supervision and mentoring to students and junior staff
What You’ll Need:
- Social Work degree or equivalent and Social Work England registration
- Experience in statutory mental health social work, particularly around hospital discharge
- Knowledge of the Care Act, Mental Capacity Act, safeguarding legislation, and best practice
- Excellent communication, decision-making, and caseload management skills
- A valid UK driving licence is desirable due to the requirement to travel across the county
Desirable (but not essential):
- AMHP or Best Interest Assessor qualification (or willingness to complete within 2 years)
