Transition Support Worker 
Ashton-under-Lyne, Tameside
06/01/2024 – 3 months initially
Monday to Friday, 8.30am – 5pm, 36 hrs/week – Hybrid 
  
Our client are seeking a Transition Support Worker to join a team based in Ashton-under-Lyne.
  • Direct work with children up to the age of 21, who have left care and who have previously lived in children’s homes, designed to offer them practical, employment, education, emotional and social help in their lives as they exit care
  • Undertake support needs analyses with young people, and develop individual plans to address their future aims and objectives
  • Link the young people into services and support appropriate to their needs (i.e.. counselling, positive activities)
  • Develop independence skills such as, budgeting, cooking, shopping, DIY, paying bills, dealing with utility providers, housing providers etc.
  • Assist young people by liaising with other agencies on their behalf
  • Ensure that each young person is equipped with skills to be able to live a successful and positive life, once they move into their own home
  • Ensure that the young people access a variety of opportunities to further develop interests and skills
  • Respond to any child protection/ adult at risk concerns as and when they arise
  • Build positive, genuine relationships with young people
  • Ensure their views and wishes are taken into consideration and advocate where needed
  • Keep and maintain records of work
  • Attend meetings and reviews where necessary
Requirements
  • Full UK Driving Licence, access to a vehicle, business insurance
  • Level 3 qualification in childcare or equivalent is desirable
  • Experience of working with adolescents 16 – 25 years, vulnerability and complex needs, behaviour needs & independence skills
  • Residential experience
  • Experience in assessment of children’s needs
  • Child and Adult DBS registered to the update service
