Job Description
Cardiff
13/01/2024 – 3 months initially
Monday to Friday, 8.30am – 5pm, 37 hrs/week
- Support assessments of care and support needs within the hospital social work team.
- Arrange and monitor appropriate services in line with individual care plans.
- Complete care plan reviews and or reassessments.
- Provide advice and practical help.
- Speak with them about any issues they may be having or are worried about.
- Liaise with voluntary and statutory agencies.
- Participate in team meetings and duty rotas.
- Undertake training and self-development and to contribute to this process in others.
- UK Driving Licence, access to a vehicle, business insurance
- Good general education to GCSE level
- Significant experience working with vulnerable adults
- Experience in assessments for care and support needs
- Enhanced DBS registered to the update service
