09/01/2024 – 3 months initially
There are various rotas available, a minimum of 18 hours, up to and including 37 hours – between the hours of 7am – 9.30pm
(all rotas include weekend, evenings and any Bank Holidays to be worked and they must be willing to work in any area of Cardiff)
Our client are seeking a Home Carer to join their Reablement and Community Resource Team.
- You will work in partnership with the adults we support to increase their independence at home
- Provide high quality care and support for service users to maximise outcomes detailed in the care plan and the service delivery plan
- This will involve support to learn or re-learn the skills necessary for independent daily living, following a period of illness or a stay in hospital
- Provide prompt support and encouragement, empowering them to exercise informed choice, considering any risk involved
- Support people in the taking of medication
- Contribute to the ongoing review of service user needs to ensure that appropriate levels of service are offered
- Monitor the wellbeing of service users and report any changes to the Home Care Manager/Home Care Co-ordinator
- Maintain Health and Safety standards and follow procedures in all aspects of the job, including manual handling, accident reporting, food hygiene
- Maintain accurate records as required by procedures
- Attend meetings and reviews where necessary
- Must be able to start immediately
- NVQ Level 2 in Health and Social Care
- Will be required to register with Social Care Wales, though this can be arranged for the successful candidates when they join the service
- Experience providing support in independent daily living
- Enhanced Adult DBS registered to the update service
