Leaving Care Worker
Lincoln
28/07/2025 – 3 months initially
Monday to Friday, 9am – 5pm, 37 hrs/week
- Direct work with children in care and care leavers age 18-25 years, to move successfully through life, secure education, training, employment, access stable accommodation, and to progress into adulthood as a happy and healthy adult
- This will involve the co-ordination, preparation and completion of pathway plans and supporting young people preparing for leaving independently into adulthood
- Ensure that young people have an appropriate plan of service provision designed to meet their individual needs and these plans are drawn up in consultation with those young people, their social workers, carers, parents and staff from relevant agencies
- Ensure that the necessary assessment and intervention is completed
- Provide information and support to young people on an individual or group basis as appropriate
- Keep in constant contact with the young people, noting process/ concerns
- Respond to need as it arises, reporting any concerns where necessary
- Assist in ensuring that all young people referred to the Service are involved in the assessment of their case
- Work alongside appropriate services to identify and meet the needs of young people
- Participate in the provision of a duty/referral service by the service and to advise young people and their referrers as part of this process
- Maintain appropriate, accurate and securely keep recording of assessments, plans and other casework done
- Keep up to date with new trends and developments in legislation, policy and practice in the areas of aftercare, accommodation, employment, benefits, and child care
- Assist in the monitoring and evaluation of the work of the Service
- Attend meetings, reviews and conferences where necessary
Requirements
- UK Driving Licence, access to a vehicle, business insurance
- Must be able to stay overnight on occasions with notice
- Demonstrate a good level of knowledge and skills in the Leaving Care arena (16 to 25)
- Experience implementing and coordinating plans
- Ability to produce and maintain accurate records and reports
- Child and Adult DBS registered to the update
