Job Description

Targeted Early Help Support Practitioner 
Barnsley (will need to travel across the borough)
Umbrella Pay- £20.18 per hour 
4 months initially 
Monday to Friday, 37 hrs/week – Hybrid
  
Our client are seeking an experienced Targeted Early Help Support Practitioner to join an Early Start and Families Service.
  • Direct work with children, young people and families, managing a caseload of up to 15 families
  • Support targeted families with children aged 0-25 years through targeted outreach/home visiting and Family support interventions as identified though the Early Help Assessment
  • Complete whole family assessments and deliver whole family plans to targeted needs
  • Identify the hard to reach and vulnerable families within the local area, in response to locality data, and promote Family Centre Services to them
  • Undertake appropriate referrals to specialist and statutory services
  • Report and record any safeguarding concerns immediately
  • Refer families to specialist services as required
  • Provide advice, support and guidance to promote learning and positive parenting skills
  • Work collaboratively with multi agency partners championing integrated services, planning and delivering collaborative working and avoiding duplication of services
  • Support in the planning of services that meet the needs of the locality
  • participate collaboratively in regular clinical supervision
  • Keep and maintain all files/records
  • Attend meetings and reviews where necessary
Requirements
  • Level 3 Diploma for the Children and Young Peoples Workforce or equivalent
  • Relevant extensive experience of working with children and families in the community within a family support context
  • Knowledge of the Early Help Assessment and associated paperwork and processes
  • Knowledge of effective evidenced based interventions utilised in the provision of interventions for children young people and families
  • Excellent written and verbal communication skills
  • Enhanced Child and Adult DBS registered onto the update service
