Job Description

Social Work Assistant – Learning Disabilities 
Day Centre, Canton
02/12/2024 – 3 months initially
Monday to Friday, 9am – 5pm, 37 hrs/week
  
Our client are seeking a Social Work Assistant to join a Supported Living, Learning Disabilities Team based in Canton.
  • Direct work with adults with learning disabilities to enable them to live closer to home
  • Assisting adults with learning disabilities to resolve major difficulties in their lives and to live as independently as possible
  • Undertake wellbeing and risk assessments while considering outcomes
  • Undertake non-complex capacity assessments
  • Make recommendations regarding packages of care to meet identified needs, considering all available resources
  • Develop, implement and review care packages
  • Report any cases that become complex and require social work input
  • Work with other agencies such as health, housing, police where needed
  • Being part of planning and facilitating moves for individuals
  • Carrying out Monitoring Visits to Supported Living Schemes
  • Assisting with the overall Monitoring of the Supported Living Contact
  • Keep and maintain accurate records
  • Attend meetings, reviews and conferences where necessary
Requirements
  • UK Driving Licence, access to a vehicle, business insurance
  • Experience of working with individuals with learning disabilities and/ or Supported Living
  • Experience in assessments and care packages
  • IT Skills and experience of using SharePoint and Excel Spreadsheets are strongly desired
  • Excellent written and verbal communication skills
  • Enhanced Adult DBS registered to the update service
