Ashton-under-Lyne, Tameside
06/01/2024 – 1 year initially
21 hours per week over 3 days – Hybrid
- Supporting young people within their home or placement
- Attend, plan and carry out reviews and home visits
- Make face to face contact with young people referred to the project, and work to engage them with the service
- Undertake support needs analyses with young people, and develop individual plans to address their future aims and objectives
- Follow and implement recommendations of reviews and Pathway Plans
- Work with the young people to help reach their goals
- Support to access positive activities, education, training, employment, housing, health support
- Direct work with others who are relevant in their lives
- Develop supportive and empowering relationships with young people to build their self-esteem, confidence, knowledge and resilience to enable them to move forward positively in their lives
- Respond appropriately to any child protection/Adult at Risk concerns as and when they arise
- Liaising with other agencies on their behalf
- Keep and maintain records of work done
- Attend meetings, reviews and forums relating to the young people where necessary
- Full UK Driving Licence, access to a vehicle, business insurance
- Minimum of a NVQ Level 3 Care of Children and Young People
- Experience of working with young people, vulnerability and complex needs, behaviour needs & independence skills
- Experience in assessment of children’s needs
- Excellent organisation skills
- Child and Adult DBS registered to the update service
