Job Description

Part-Time Leaving Care Personal Advisor
Ashton-under-Lyne, Tameside
06/01/2024 – 1 year initially
21 hours per week over 3 days – Hybrid 
  • Supporting young people within their home or placement
  • Attend, plan and carry out reviews and home visits
  • Make face to face contact with young people referred to the project, and work to engage them with the service
  • Undertake support needs analyses with young people, and develop individual plans to address their future aims and objectives
  • Follow and implement recommendations of reviews and Pathway Plans
  • Work with the young people to help reach their goals
  • Support to access positive activities, education, training, employment, housing, health support
  • Direct work with others who are relevant in their lives
  • Develop supportive and empowering relationships with young people to build their self-esteem, confidence, knowledge and resilience to enable them to move forward positively in their lives
  • Respond appropriately to any child protection/Adult at Risk concerns as and when they arise
  • Liaising with other agencies on their behalf
  • Keep and maintain records of work done
  • Attend meetings, reviews and forums relating to the young people where necessary
Requirements
  • Full UK Driving Licence, access to a vehicle, business insurance
  • Minimum of a NVQ Level 3 Care of Children and Young People
  • Experience of working with young people, vulnerability and complex needs, behaviour needs & independence skills
  • Experience in assessment of children’s needs
  • Excellent organisation skills
  • Child and Adult DBS registered to the update service
